Position Overview:


This position will lead the community health assessment (CHA) and community health implementation plan (CHIP) initiative by facilitating community meetings, establishing work groups, and working with consultants.

Coordinates CHA/CHIP forums and helps collect and analyze demographic and health data. Works collaboratively with partners to prioritize, develop, and implement the CHIP.

Shares and presents CHA findings and CHIP goals with partner organizations and community members; and develops monitoring tools to track progresses, outcomes, or impacts.-

  • Experience conducting literature reviews and summarizing findings.
  • Experience with community engagement techniques and the planning process.
  • Experience in research, statistical analysis, focus groups, community forums, and survey techniques.
  • Experience collaboratively working with community members and partner organizations.
  • Experience with providing oral and written communication, including highprofile presentations.
  • Ability to travel to more than one work location.

Duties, Functions and Responsibilities

  • Assists in planning, executing and closing projects in a variety of operational and/or administrative situations related to specific assignments/areas.- 2. Develops and maintains databases to support data analysis and research findings for business projects.- 3. Assists in preparation of recommendations for implementation of new or revised policies, procedures, and/or process changes/improvements.- 4. Write reports that summarize findings, data collection techniques, implementation options, etc.- 5. Establishes project schedules using project planning software/tools, updates and maintains project status reports. Communicate project/process status.- 6. Conduct research, analyze trends, determine accuracy and relevancy of data/information, and use sounds judgment to evaluate alternatives.- 7. Facilitates and documents meetings, action items, and processes using flowcharting methodologies.- 8. Maintains positive working relationships with managers, supe
