    We are seeking a Case Management Registered Nurse PRN to join our team You will be responsible for the assessment, reassessments, and discharge planning for assigned clients.

    Responsibilities:

    • Administer home care assessments for seniors, injured, and or disabled clients
    • Determine level of care and develop client care plans
    • Monitor and report changes in client symptoms or behavior
    • Communicate with collaborating physicians or specialists regarding client care
    • Educate clients about health maintenance and disease prevention
    • Facilitate referrals to other healthcare professionals and medical facilities
    • Maintain accurate client medical records
    • Provide advice and emotional support to clients and their family members

    Qualifications:

    • Previous experience in nursing or other medical fields
    • Familiarity with medical software and equipment
    • Ability to build rapport with clients
    • Strong problem solving and critical thinking skills
    • Ability to thrive in a fast-paced environment
