About the Position:
Matrix providers spend up to an hour reviewing a member's health, medical history, and social environment during visits. This in-depth assessment allows for better care and can address multiple health gaps in one visit.

Matrix Provider – What to Expect:
Nurse Practitioner - Part Time/PRN - Home Risk Assessments
Type: Part Time/PRN - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 (depending on county) per assessment
Location: Home, SNF, Telehealth, and various environments in Lorain, Ohio
Hours: Flexible scheduling including days, evenings, nights, and weekends
Benefits: PRN/Part Time providers receive non-benefited roles with 401(k) and limited Voluntary Benefits available
Mileage reimbursement plan
