- Role: Personal Service Coordinator/Intensive Case Manager
- Hours: 35 hours per week
- Compensation: $24.00 per hour
- Provide high-quality Intensive Case Management Services to clients eligible for permanent housing through the DHS Housing for Health program.
- Assist individuals at various stages of the housing stabilization process.
- Conduct intake and enrollment activities with eligible clients.
- Help clients access essential services and address immediate needs.
- Ensure clients are linked to health and mental health services as needed.
- Provide housing location services and educate clients on tenant rights.
- Document all program activities accurately and promptly.
- Participate in the On-call Crisis Prevention rotation schedule.
- Engage in continuous professional development activities.
- High school diploma or GED with 2 years' relevant experience preferred.
- Proficiency in Microsoft Word, Excel, and Outlook.
- Experience working with clients with mental illness and substance use disorders.
- Flexible work schedule, including nights, weekends, and holidays.
- Valid driver's license and reliable transportation.
- TB clearance, COVID-19 vaccination, and proof of automobile insurance required.
