Overview:
We are seeking a dedicated and compassionate Mental Health Case Manager to join our team.
As a Mental Health Case Manager, you will play a crucial role in providing support and assistance to individuals with mental health conditions.
Your primary responsibility will be to coordinate and monitor the care and services provided to clients, ensuring their well-being and helping them achieve their treatment goals.
Duties:
- Conduct comprehensive assessments of clients' mental health needs
- Develop individualized treatment plans in collaboration with clients and other healthcare professionals
- Coordinate and monitor the implementation of treatment plans, including therapy sessions, medication management, and other necessary interventions
- Advocate for clients' rights and access to appropriate services
- Provide crisis intervention and support during emergency situations
- Collaborate with community resources and agencies to ensure clients have access to necessary support services
- Maintain accurate and uptodate client records, including progress notes, treatment plans, and other relevant documentation
- Assist clients in navigating the healthcare system and accessing available resources
- Monitor clients' progress towards treatment goals and make adjustments to treatment plans as needed
Experience:
- Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related field
- Previous experience working in a mental health setting is preferred
- Strong communication skills, both verbal and written
- Excellent organizational skills with the ability to manage multiple tasks simultaneously
- Proficient in using computer software for file management, calendar management, and clerical tasks
- Knowledge of community resources and support services for individuals with mental health conditions
- Ability to work independently as well as collaboratively within a team

Microsoft Office: 1 year (preferred)
