- High school diploma or equivalent.
- Ability to communicate accurately in writing.
- Ability to work well as a team member.
- Prior experience working with individuals with developmental disabilities.
- Ability to transfer individuals with physical disabilities.
- Possession of a clean, valid NYS Driver's License.
- Implement various services in compliance with OPWDD Day Habilitation guidelines.
- Assist with loading and unloading individuals from vans
- Physically transfer program participants during ADL and positioning activities
- Complete general care (i.e. feeding, toileting, jacketing, etc.) of individuals.
- Administration of tube feedings/fluids as prescribed by medical staff.
- Attend community activities with program participants and other DSPs.
- Implement Day Habilitation plans. Through various formal and informal assessment techniques, evaluate the individual's strengths and weaknesses to determine valued outcomes.
- Maintain individual records daily and submit weekly and monthly documentation according to established timelines.
- Expeditiously report any sickness, injury or seizure to medical authorities. Complete necessary paperwork – i.e., incident reports, seizure reports.
- Participate in cleaning and general maintenance of adaptive equipment and personal possessions of individuals in program.
- Participate in mandatory staff meetings and in-service training.
