    Job Title: LPN Assisted Living Facility in Westchester

    About the Company: Protingent Staffing offers a fantastic Direct Hire opportunity for an LPN at an Assisted Living Facility in Westchester, NY.

    Job Description:
    • Perform various tasks within the realm of health counseling and provision of supportive care under the guidance of a registered professional nurse.
    • Communicate initial findings to the home care coordinator, RN, or supervisor.
    • Follow the Plan of Care in coordination with the family, physician, or authorized practitioner.
    • Deliver nursing services, treatments, and preventive procedures as per the Plan of Treatment.
    • Assess and address changes in patient condition promptly.
    • Conduct nutritional assessments under the supervision of the Director of Nutrition.
    • Provide timely reporting of patient information to the Primary RN or Case Manager.
    • Submit accurate visit records and vouchers.
    Job Requirements:
    Requirements:
    • NYS LPN License.
    • CHHA Experience preferred.
    • Background in home care is advantageous.
    • New graduates are encouraged to apply.
    Position Details:
    • Direct Hire role.
    • Work schedule: Fri, Sat, Sun, & Mon from 7AM to 5PM.
    • Compensation: $35/hour
    • Location: Westchester, NY.
