- Associate's degree that is relative and focused in health, education or welfare of children or comparable experience
- Three years of direct client service experience and one year administrative experience in a health or human service program
- Ability to interact positively with others of various backgrounds
- Willingness to accept the mission and goals of the Margaret B. Shipley Child Health Clinic, Inc.
- Completes admission for new patients establishing care, verifies insurances, establishes initial appointments and assesses social determinates of health (SDOH)
- Coordinates Clinic's Reach Out and Read program and Cribs for Kids program
- Prioritizes and processes release of record requests for Social Security office, CPS, ODJFS, etc.
- Measures and assesses all new families for SDOH based on screening assessment
- Coordinates appropriate referrals to community services as it pertains to social needs of the family and follows up
- Provides patients/families with age-appropriate educational literature to improve health outcomes
- Follows up on referrals made by providers and eases family's barriers to making/keeping appointments
- Communicates with ODJFS and other programs pertaining to foster care and CPS
- Communicates with provider(s) and clinical staff, and provides written documentation regarding referrals, SDOH, progress and community service involvement in EMR
- Communicates with Medicaid payers on incentive programs in collaboration with Executive Director
- Coordinates patient care gap data and provides strategy to outreach team as it pertains to population health and continuity of care working to decrease health inequities
- Provides guidance and assistance to the clinical staff
- Provides other tasks as requested by Executive Director or Providers
- Participates in continuing education

RESPONSIBILITIES
The Care Coordinator is responsible for the coordination of care at the Clinic. The Care Coordination program is a structured system to ensure patients/families receive comprehensive and continuous care to improve patient outcomes and health. The Care Coordinator may function as the liaison with other agencies and team leaders; responds to the clinic Pediatrician and is directly responsible to the Executive Director.
