This position involves close coordination with the Housing Authority of Cook County (HACC) Property Manager to identify individuals and families in need of community resource education, linkage to crisis services, and Integrated Healthcare Services as necessary.


Essential Duties and Responsibilities:

  • Initiate and coordinate referral to Grand Prairie Services Intake Department.
  • Advocate on client's behalf for access to community resources.
  • Promote active participation in decision making and self-advocacy in all service aspects and recovery.
  • Provide community support services to clients ensuring linkage and access to necessary resources and services.
  • Offer linkage case management, counseling, and other essential resources when appropriate.
  • Participate in scheduled team meetings.

Education and/or Experience:
Bachelor's degree (B.A.) from a four-year college or university in psychology, counseling, or social work. Meets eligibility requirements as a Mental Health Professional as defined in Medicaid Rule 132. Must have proficient typing and computer skills.


Language Skills:
Ability to read and interpret documents like policy manuals, operating and maintenance instructions, and procedure manuals. Can write routine reports and correspondence. Able to speak effectively before groups of clients or employees of the organization.


Mathematical Skills:
Ability to perform basic math operations and interpret data effectively.


Reasoning Ability:
Capable of defining problems, collecting data, establishing facts, and drawing valid conclusions.


Certificates, Licenses, Registrations:
None required.


Physical Demands:


The physical demands required include sitting and communication. This job may occasionally involve standing, walking, reaching, climbing, or crouching. Specific vision abilities are needed, and the employee must operate an automobile.



Work Environment:


The work environment includes exposure to outside weather conditions and providing direct services in the consumer's natural setting. The noise level is usually moderate.
