- High School Education is required.
- Obtain and maintain medication certification.
- Good interpersonal relationship and communication skills.
- Maturity, good judgment, and ability to handle confidential information.
- Must be proficient, or become proficient, and maintain proficiency in first aid, CPR, PMT and other resident specific medical and habilitative needs. Attend all required in-services and staff meetings. Review and follow all agency policies and procedures.
- Competent in basic computer skills, experienced with electronic health records is preferred.
- Valid Connecticut Driver's License and eligibility for Network, Inc.'s Auto Insurance.
- Must be physically able to assist participants when necessary (i.e. lifting, using approved adaptive equipment, using approved physical restraint techniques).
- Must be proficient, or become proficient, and maintain proficiency in the electronic health record system THERAP.
- Assists in assessing participant's leisure and social skills.
- Responsible for ensuring training strategies are implemented and data collected.
- Reports safety concerns to supervisor immediately.
- Adheres to State of Connecticut Department of Developmental Services.
- Assist in activity planning and identification of community resources and opportunities.
- Assist in developing a social network and in overcoming barriers to access and participation in the community.
- Participate in on-going in-services and staff meetings.
- Observes all clients' rights.
- Works as an active and constructive team member in all duties.
- Maintains client confidentiality.
- May be requested to cover coworkers' vacation, sick, or other planned/unplanned absences from work as well as additional emergency staffing including being frozen-in on shift.
- Follow all aspects of client behavior plans and provide direct care and assistance to program participants.
- Responsible for medication administration in accordance with regulations.
- Responsible for safeguarding and proper handling of residents' belongings and funds on the premises.
- Documents appropriate resident and residence electronic health records including:
- Periodic resident specific status assessment.
- Daily resident specific task analysis charts.
- Other daily resident specific records as directed.
- Ongoing inter-staff communication in the T-logs.
- Medication administration documentation.
- Resident attendance.
- Ongoing residence maintenance records as directed.
- Other records as directed.

Job DescriptionSCOPE OF RESPONSIBIITY:
To provide a variety of activity and social skill training to participants in the Community Experience Program. To serve as a liaison to community resources and assist the individual in developing a social network and other supports.
WORKING RELATIONSHIP:
The Direct Support Staff reports directly to the Manager or the Lead Direct Support Staff on duty for all aspects of the job responsibilities. Develops and maintains relationships with the following:
