JOB SUMMARY
This position is responsible for providing patient care in a safe and effective manner.

Responsibilities include, but are not limited to:
delegation of tasks and duties to team

members; synthesizing patient information; assessment and care appropriate to the age of the patients served, with appropriate nursing interventions; creating and updating the patient plan of care.

Ensures that all physician orders are carried out appropriately are also responsibilities of the Registered Nurse. Patient and family education are provided by the Registered Nurse. Integration of patient care standards are incorporated into all clinical activities.

TELEPHONE SKILLS - Demonstrates hospital telephone etiquette by answering the phones promptly, courteously and identifying department and self. POLICIES/PROCEDURES- Is aware of and follows all hospital and department policies and procedures.
TEAM WORK-Promotes positive co-worker relationships through team work and cooperation. Understands and supports the needs of individuals in other departments and promotes conflict resolution. Shares information, work knowledge and experience with coworkers, physicians and students in a non-threatening manner. Offers assistance and promptly responds to requests.
PRODUCTIVITY -Maintains high productivity and performs efficiently regardless of whether the work volume is high or low. Appropriately prioritizes work activities. Interviews patient, family, significant others to formulate a plan of care.
Documentation is accurate, complete and legible.

Observes, identifies and collects data to identify nursing care needs of the patient and family.
Correlates the medical plan of care with the nursing plan of care.
Identifies and reports to the appropriate person(s) abnormal diagnostic data and clinical changes in the patient's condition


PLANNING:
Identified patient problems and nursing care needs of the patient.
Identifies the need to involve other disciplines in the patient's care and communicates those needs appropriately.
Initiates discharge planning upon admission.

IMPLEMENTATION
Performs common nursing procedures as defined in the Policy and Procedure Manual.
|Validates and implements physician orders in a timely manner.
Provides patient and family teaching.
Provides for the safety, cleanliness and comfort of the patient.
Rounding every two hours for patient care and safety.
Initiates emergency procedures as needed within hospital policy.

EVALUATION:
Evaluates the patient's response to nursing care.
Updates and revises plan of care as patient's needs change.
Appropriately communicates changes and care given to other members of the health care team.

PROFESSIONAL GROWTH AND RESPONSIBILITIES

Assumes leadership role and delegates nursing care appropriately per scope of practice of team members and guides them based on their skills and Maintains awareness of legal aspects of hospital nursing.

Assists in orientation and/or acts as a preceptor for new personnel.
Integrates Commitment to Quality Initiatives and Core Measure Practice Standards to improve patient outcomes.
Completes computerized system acuity and other functions as assigned:

UNIT SPECIFIC IF APPLICABLE:
Safely administers chemotherapy, demonstrating knowledge of critical lab values and care of extraversion.
Manages the care of the patient undergoing radiation-type treatment.
Identifies and follows through on the needs of the patient/family in a death/dying situation.
Manages all vascular access devices, including implanted ports, tunneled catheters and PICCs.
Manages patients with potential/actual impairment of skin integrity, demonstrating knowledge of use of skin care products, staging and documentation. Manages the care of patients with chest tubes, PCA pumps, and CPM machines.
Demonstrates understanding of cardiac medications and administration.
Interprets telemetry patient rhythm and documents appropriately.
Completes assignment with mínimal use of overtime.
Attends at least 80% of team meetings. Accountable for information presented at team meetings.

Demonstrates the knowledge and skill necessary to provide care appropriate to the age of the patients served.

Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to asses data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed by the following patient groups: Adolescents , Adults , Older Adults (65+).
