Description
This position requires that the incumbent work with clients to connect them with supportive community engagement and employment services. Incumbent will participate in collaborative client assessment and treatment planning. Incumbent will provide services in the client's home and in the community to support integration and stabilization. All services are provided as part of a comprehensive treatment team including doctor, nurses, therapists and case managers.

This position may involve weekend duties to maximize community integration for clients, as well as occasional scheduled duties beyond traditional work hours (shared crisis consultation and rotating holiday coverage).

QUALIFICATIONS:


  • Master's degree in a relevant field required
  • LPC or LCSW eligible required, licensed preferred
  • Professional and clinical experience in the provision of vocational rehabilitation required

ADDITIONAL REQUIREMENTS:


  • Must possess a valid Driver's License, good driving record, and the ability to provide own transportation
  • May be exposed to allergenic environments in client's homes, including extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
  • May be exposed to clients with infectious diseases
  • Must be able to drive for long periods of time to visit clients in and out of the catchment area as needed
  • Must be able to walk distances, climb and descend stairs, maneuver in and out of agency vehicles
  • Prolonged periods sitting at a desk and working on a computer
  • Must be able to lift/push/pull up to 25 pounds at times

SALARY:
Starting at $57,916.00
