- Conduct regular team meetings, training sessions, and performance evaluations to enhance staff development.
- Develop and implement care coordination protocols and workflows to optimize patient outcomes.
- Serve as an advocate for patients, ensuring their needs and preferences are communicated and addressed within the healthcare system.
- Collaborate with healthcare providers and community resources to support patients in navigating the healthcare system.
- Monitor and evaluate the effectiveness of care coordination interventions, implementing Axeium & Relevant improvements as needed.
- Collaborate with the Quality Improvement team to ensure compliance with FQHC Look-Alike standards and regulations.
- Collaborate with other departmental supervisors and healthcare providers to facilitate interdisciplinary communication and collaboration.
- Participate in regular meetings to discuss patient care, quality improvement initiatives, and overall health center operations.
- Oversee the care coordinating team that coordinates care for our patients.
- Analyze data and generate reports to track key performance indicators and outcomes related to care coordination.
- Develop and deliver ongoing training for Care Coordinators to enhance their skills in patient engagement, communication, and care coordination.
- Bachelor's degree in social work, nursing, health, human services or closely related field; Master's Degree (preferred.)
- Minimum four years of experience in the human services field. Additional experience may be substituted for the academic requirements identified above.
- Knowledge and experience in working with diverse communities in the Orange County, CA area.
- Thorough understanding of Medicaid Waiver systems and community resources.
- Bilingual English/Spanish required
- Minimum of one year in a supervisory role (preferred.)
- Knowledge of program planning, implementation and evaluation; experience evaluating client needs, completing assessments and plans of care.
- Knowledge of or willingness to learn about the various aspects of Care Coordination, QA/QI, PCMH and any related programmatic or regulatory needs.
- Sensitivity to clients of all cultures, diversity and backgrounds.
- Ability to direct, supervise and evaluate personnel
- Strong written and verbal communication skills
- Exceptional interpersonal, organizational, and planning skills
