- Complete intake and needs assessments with each YYA, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, legal issues, health, and daily living skills.
- Interview and evaluate clients to formulate Individualized Service Plans based on Program requirements.
- Assist YYA's in securing permanent housing by making full use of all available housing opportunities including the private rental market and reunification where applicable
- Work closely with YYA to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process.
- Maintain compliance standards with Youth Services programming.
- Help YYA build self-sufficiency plans.
- Assist YYA to transition to self-advocacy roles.
- Provide emergency response management.
- Establish and monitor goals.
- Work collaboratively with interdisciplinary team of collateral providers.
- Maintain accurate case records.
- Prior related experience required
- High School Diploma or Equivalent required
- Prior assessment, advocacy and case management work in a Human Service field.
- Knowledge and experience working with low income, culturally diverse, and at-risk individuals.
- Demonstrated knowledge of community resources available to low-income individuals.
- Excellent customer service, organizational, oral and written communication skills.
- Ability to manage multiple priorities and work in a fast-paced environment.
- Proficient computer skills required in basic computer software and data collection systems.
- Excellent crisis management skills.
- Ability to maintain accurate records and confidentiality of information.
- Excellent attention to detail.
- Valid Driver's License
- Bilingual (English/Spanish, English/ Haitian-Creole, English/Khmer, English/Portuguese) preferred.

Youth Advocate Shelter Case Manager
Location: Lowell
We are seeking aYouth Advocate Shelter Case Managertoprovide services to youth placed within the 24/7 Crisis Transitional Youth Shelter. TheYouth Advocate Shelter Case Manager is responsible for providing case management services to Youth and Young Adults (YYA) ages 18-24, using a trauma informed approach that includes: developing monthly Rehousing Plans, referrals to community resources, advocacy, assessments, and crisis intervention, with the goal of placing YYA into permeant housing.
