Case Manager Job Description


Job Summary:


The Case Manager plays a crucial role in our reentry program, providing individualized support and guidance to male clients recently released from prison or jail.

This position requires a compassionate, resourceful, and highly organized professional who will work closely with clients to create and implement customized reentry plans.

Road to Renewal is a reentry program dedicated to supporting male clients in their successful transition back into the community out of state and local court sanctioned institutions.

We provide comprehensive services to empower clients towards a positive, meaningful, and successful future.


Responsibilities:


  • Conduct thorough assessments of clients' strengths, needs, and barriers to successful reintegration into the community.
  • Collaborate with clients to develop individualized reentry plans, setting realistic goals and objectives.
  • Connect clients with a range of community resources, including housing, employment, healthcare, substance abuse treatment, mental health services, documentation needs and legal assistance.
  • Provide immediate support and interventions during crisis situations, ensuring clients' safety and wellbeing.
  • Lead group sessions focusing on various aspects of recovery, including relapse prevention, coping skills, anger management, and life skills.
  • Maintain accurate and uptodate case records, documenting client progress, interventions, and outcomes in accordance with program and regulatory standards.
  • Assist and support the Community Support Specialist with daily check ins with residents such as calling to verify location or to verify employment
  • Maintain consistent communication with client's probation/parole team
  • Adherence to all expected contract and organization regulations
  • Oversight of medication administration as needed
  • Adhere to all Programmatic Policies and Procedures, including Driving Policy.
  • Occasional transportation of residents is possible.
  • Adhere to LHI employee handbook rules.
  • Appropriate utilization of oncall procedures.
  • Responsible for the immediate notification of safety/health/legal issues happening in the program (resident or programmatic), including completing an Internal or Critical Incident Report form.
  • Complete medication training with the Program Director.
  • Adhere to all programmatic medication policies and procedures.
  • Observe and document residents' selfadministration of medications.
  • Complete daily medication counts.
  • Assist with Program Monitor Job duties as needed
  • Other duties as assigned by APD and PD

- Must pass a drug screening upon hire and annually

- Must pass a CORI background check

Advocacy and Empowerment:


  • Advocate for clients to ensure their access to necessary services, resources, and opportunities for personal growth and selfimprovement within the community.
  • Facilitate workshops and sessions to enhance clients' life skills, including budgeting, time management, communication, and conflict resolution.
  • Work closely with the Assistant Program Director, Program Director, and other team members to ensure a coordinated and holistic approach to client support.
  • Demonstrate cultural sensitivity and competence in interactions with clients, recognizing and respecting diverse backgrounds and experiences.

Compliance and Reporting:


  • Adhere to all company and program policies, procedures, and relevant legal and ethical standards and ensure timely and accurate reporting as required.
  • Understand and strictly adhere to State and Federal HIPPA and PREA guidelines.
  • Understand and strictly adhere to mandated reporting guidelines

Knowledge and Skill Requirements:


  • Bachelor's degree in social work, psychology, counseling, or a related field (master's degree preferred).
  • Previous experience in case management or working with individuals involved in the criminal justice system or reentry programs is highly desirable.
  • Knowledge of substance abuse treatment, mental health, and traumainformed care principles.
  • Strong organizational, communication, and problemsolving skills.
  • Ability to work independently and as part of a collaborative team.

Physical demands

  • Must be able to sit or stand for an extended period.
  • Must occasionally lift and/or move up to 25 lbs.
