JOB DESCRIPTION: PATIENT NAVIGATOR 

REPORTS TO:  Community Wellness & Outreach Nurse Manager 

STATUS: Full-Time Non-Exempt  

POSITION SUMMARY:  This position guides patients by supporting patients to anticipate, address, and overcome barriers to care, while guiding patients through the healthcare delivery systems that provide their care. The Patient Navigator workings closely with the Indian Health Center care teams to ensure high patient satisfaction. Additionally, this position provides administrative and program support for Health Education Program and CWO RN Manager.   

DUTIES & RESPONSIBILITIES: 
• Assist CWO RN Manager in overseeing all program office routines to ensure a well-organized delivery of services
• Under supervision of the CWO RN Manager, assists in health screenings, education, and case management support
• Provides support for tracking and reporting activities of patient outcomes
• Document client encounters through electronic charting methods
• Assist the CWO RN Manager in preparing and organizing materials for health education program meetings as assigned, i.e., take minutes, coordinate logistics, etc.
• Responsible for organizing CWO (Community Wellness & Outreach) health education activities by updating flyers, reading material, ordering supplies, making contacts, mail outs, etc.
• Maintains supplies for health education program
• Provides general information regarding referrals for walk in and telephone clients about IHC or CWO services
• Seek current information about available patient resources
• Maintain current disease information for patients
• Maintain communication with patients and the healthcare team about patient healthcare needs
• Contact patients regularly to assist in accessing healthcare appointments, including IHC appointments and outside specialty appointments
• Provide health information and screening information
• Empower and coach patients to self-navigate the healthcare system
• Assist patients in understanding care plans and instructions and helps patients actualize health management plans and goals.
• Assist patient with barriers to care including but not limited to scheduling appointments, transportation needs, and access to community resources unknown or otherwise unavailable to patients without appropriate referrals.
• Document activities, plans, and results in an effective manner and provides reports to the CWO RN Manager
• Receive patient requests for assistance and refers patient to appropriate member of the care team for resolution, unless Patient Navigator can resolve on their own within the scope of the position.
• Provide general clerical duties such as filing, answering phones, taking messages, faxing, mailing, copying, data entry, and ordering office and medical supplies for all health education staff
• Coordinate interdepartmental referral process
• Participate in IHC sponsored programs, activities and other community events
• Participate in Indian Health Service (IHS), Indian Health Center, and other appropriate trainings and webinars
• Promptly reports any issues, problems, or patient concerns to the CWO RN Manager or Director of Clinical Services
• Develop professional relationships with individuals of the patient's care team and organizations that provide patient services or resources
• Report to other clinic sites as needed
• Maintains confidentiality and security of medical records and data entry
• Follows HIPAA policies and procedures
• Performs other duties as assigned  

REQUIRED QUALIFICATIONS, KNOWLEDGE AND ABILITIES: 
• High school graduate or GED with a minimum of one-year work experience
• Flexibility, initiative, and reliability
• Excellent customer service skills required
• Spanish language skills preferred
• Knowledge of and ability to relate to the American Indian community and other minority populations
• Ability to communicate both verbally and in writing
• Demonstrate empathy, compassion, and understanding for patients
• Maintain professional relationships with individuals of the patient's care team and organizations that provide patient services or resources
• Ability to use all Microsoft and Windows computer applications
• Possession of a valid California Driver License, automobile insurance, and a clean driving record
• Ability to maintain strict confidentiality
