Description

A large and busy home care agency in Upper Manhattan is looking for an experienced HOME CARE COORDINATOR to work in SOUTHERN BLVD or HARLEM.

Responsibilities include:

Scheduling caregivers to provide service for clients
Interacting with families, clients and caregivers about services (either in person or on the phone)
Providing clients and caregivers with accurate schedules
Followingup with clients and caregivers regarding coverage, callouts, and cancellation of services
Maintaining open and current communication with management team of changes in status of clients and caregivers
Ensuring effective coordination and communication of activities
Actively participating in regular staff meetings to provide guidance on any changes with staffing requirements/needs
Maintaining confidentiality regarding all aspects of job responsibilities and compliance with HIPAA Privacy and Security Rules

Requirements are as follows:

MUST HAVE PREVIOUS HOME CARE STAFFING EXPERIENCE_

Handson knowledge of HHAeXchange is a
PLUS
Ability to problem solve challenging and complex situations that involve family members/clients, care providers and referral sources
Excellent time management and organizational skills
Ability to prioritize own work load and quickly adjust priorities as required
Excellent customer service skills including the ability to effectively with a variety of personality types

Must be fluent in Spanish_
