    Job Summary:
    We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team.

    As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for individuals in need.

    This is a rewarding position that allows you to make a positive impact on the lives of patients and their families.


    Duties:

    • Conduct patient assessments to determine their healthcare needs and develop individualized care plans
    • Coordinate and monitor patient care throughout the healthcare continuum
    • Collaborate with healthcare professionals to ensure the delivery of highquality care
    • Educate patients and their families on disease management, treatment options, and available resources
    • Advocate for patients' rights and ensure their privacy and confidentiality in accordance with HIPAA regulations
    • Utilize knowledge of pediatrics, ER, critical care, and primary care to provide specialized care as needed
    • Maintain accurate and uptodate patient records and documentation
    • Stay informed about current medical practices, guidelines, and regulations
    • Collaborate with managed care organizations to facilitate appropriate utilization of resources
