- Provide targeted case management services to individuals with mental health and substance abuse disorders
- Coordinate care, engage in care planning, and ensure access to necessary services
- Participate in quality improvement activities and projects
- Complete data reports for program review
- Prioritize outreach to the homeless population
- Attend staff meetings and case conferences regularly
- Build relationships with patients and providers to ensure appointment attendance
- Maintain regular contact with clients and respond to complaints or grievances
- Engage in outreach activities via phone, electronic methods, or field work
- Provide health information, support, and coaching to prevent complications
- Promote linkage development in patient care areas and monitor effectiveness
- Communicate with Supervisor on program operations
- Ensure follow-up care and client engagement in the community
- Participate in professional training and development
- Complete required certifications in mental health/addictions treatment
- High School Diploma or GED required
- Associate Degree and Bachelor's degree preferred
- Minimum of six years of case management experience
- Mandated reporter training/certificate required
