    About This Career Opportunity

    Health Care Coordinator Role:

  • Answering directly to the Resident Services Director, you will be responsible for both providing direct care to residents and overseeing medication administration in alignment with the care strategy.
  • Your duties will involve training Medication Assistants, supervising the Medication Program, ensuring the accuracy of medication distribution systems, and guaranteeing medication dispensation complies with physician instructions.
  • It will be your responsibility to oversee that all essential documentation is completed to meet regulatory standards and organizational policies.
  • You will assist in the admission and discharge processes, conducting admission assessments, confirming completion of medical records, and validating medical orders.
  • Conducting resident assessments in case of condition changes and collaborating with the relevant medical team for required interventions and family notifications will also fall under your purview.
  • Collaborate closely with the Resident Services Director and Resident Assistants to ensure that individual resident care needs are being adequately addressed.
  • Are You Qualified?

    Here's what you need:

  • Current California RN or LVN license is mandatory.
  • A minimum of 2 years' experience in long-term care, home care, or a related field is required. Prior exposure to acute/skilled nursing communities is a definite plus.
  • Proficiency in English verbal and written communication, coupled with the ability to excel in a diverse cultural environment, is essential.
  • Demonstrated attention to detail in all tasks.
  • Working knowledge of computers, typing skills, and the capability to effectively navigate computerized Point of Care systems and other related platforms is crucial.
