    Salary: $62,500 per year (based on 40 hrs/week); $30 per hour

    Reports To: Director of Clinic Operations

    Job Summary:

    The Care Coordinator plays a crucial role in the medical support team by facilitating referrals for specialty care, assisting with insurance authorizations, and supporting clients in navigating healthcare systems and accessing community resources. This role involves case management responsibilities, including appointment accompaniments, enrolling clients in support services, and assisting with identity document updates.

    The Care Coordinator serves as a key contact for clients seeking gender-affirming surgeries and is responsible for ensuring sensitive and non-judgmental patient care.



    Key Responsibilities:
    • Submit referrals for specialty care and facilitate insurance approvals
    • Assist clients in scheduling and attending specialty care appointments
    • Coordinate gender-affirming surgeries process and support peri-operatively
    • Follow up on missed appointments and provide necessary support services
    • Help clients access social support services
    • Participate in care planning and coordination with interdisciplinary team

    Education, Knowledge, Skills:
    • Minimum one year healthcare experience

    • Professional, calm, and warm demeanor


    Preferred:
