Responsibilities:

Conduct initial nursing evaluations for hospice services eligibility within forty-eight (48) hours of referral
Assess and address the physical, psychosocial, and environmental needs of patients and families regularly
Develop and manage individualized care plans
Keep detailed records of patient's condition, progress, and response to care
Collaborate with the patient, family, and healthcare team in daily care provision
Train and supervise patients and families in self-care practices when appropriate
Maintain accurate clinical notes for reference and continuity of care

Requirements:

Minimum of one year of clinical experience
Active RN license in the state of practice with at least one year of clinical experience
Current CPR Certification
Valid driver's license, reliable vehicle insurance, and access to transportation

Additional Requirements by State:

In LA: Two years of full-time RN experience or two years of full-time hospice care experience as an LPN can substitute for RN experience
