    JOB SUMMARY:
    The facility-based APP is responsible for providing direct patient care services within the Post-Acute Care setting.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:
    • Conduct clinical care visits and re-evaluations
    • Evaluate and treat chronic illnesses
    • Review medications and diagnostic tests
    • Coordinate care with collaborating physicians
    • Perform Annual Wellness Visits and more

    QUALIFICATIONS:
    • Valid license and board certification
    • Masters Degree in Nursing or Physician Assistant Studies
    • Strong leadership and interpersonal skills
    • Excellent written and oral communication skills
    • Ability to work effectively in a team
