What will you do?
As a Care Manager at Aspire Health Partners, your job purpose is to respond,de-escalate, provide peer support, and directly connect individualsexperiencing mental health related crisis in our community to the services theyneed.
This position will work in teamsof two (one licensed clinician and one crisis support case manager) to beavailable for immediate response to dispatch of appropriate mentalhealth-related calls.
Essential functions include:
1.
Respond both via telehealth and face-to-facein the community to identified mental health crisis calls.
2.
Utilize assessment and interventiontechniques, to provide short term crisis support/counseling and to connect theclient to appropriate Aspire and/or Central Florida community resources.
Formulating an appropriate clinicaldisposition for the client in need of services.
4.
Additional internal functions includeparticipation in staff meetings, interdepartmental communication andconsultation, and community engagement with stakeholders and resources.
5.
Operational duties, such as:
a.
Generating additional documentation, datacollection, reports, and diagnostics when necessary
b.
Collaborating with outcome evaluationprocesses and/or any other tasks requested by supervisor.
Qualifications
MinimumBachelor's Degree in the Human Services field and experience working withadults with mental health or substance use disorders.
Pass a Level IIBackground clearance
Pass a MotorVehicle Records review
