This role of Patient Care Manager involves overseeing and organizing clinical services, delivering nursing care to patients at their homes as per physician's orders and legal requirements. Responsibilities include:

• Providing clinical services within the state's nursing practice regulations and the care plan
• Managing referrals, assigning clinicians, assessing patient orders, and scheduling care visits
• Coordinating patient home health benefits, insurance approvals, and medical necessity determinations
• Ensuring personalized, appropriate, and physician-directed care for patients
• Reviewing assessments and care plans daily and providing recommendations to clinicians

Education & Experience Requirements:

• Active RN license in the practicing state
• Valid CPR certification
• Driver's License, vehicle insurance, and access to reliable transportation
