  • Over the years the hospital has continually expanded to meet the needs of the community, adding an upscale obstetrics and Level II Neonatal Intensive Care Unit at The Dr. P. Phillips Baby Place, cancer care at the AdventHealth Cancer Institute, Winter Park, and stateoftheart surgery, recovery and rehabilitation at the AdventHealth Orthopedic Institute

The role you'll contribute:

Coordinates appropriate social work and case management services to patients and their immediate families. Serves as the client advocate and assists in the coordination of the rehabilitation process from admission to discharge.

Adheres to AdventHealth Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies and accrediting bodies.


The expertise and experiences you'll need to succeed:


Minimum Qualifications:


LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:


  • If RN, current valid State of Florida or multi state license as a Registered Nurse

EDUCATION AND EXPERIENCE REQUIRED:


  • RN or Bachelor's degree in Rehabilitation or Social Services

KNOWLEDGE AND SKILLS REQUIRED:


  • Entrylevel professional competency as defined in competencies departmental policy and commitment to patient and partnersoriented practice
  • Strong organizational and communication skills

Preferred Qualifications:


  • MSW
  • Experience in Rehabilitation
  • Basic computer skills to include Outlook, Microsoft Office and Windows

  • Evaluates, re-evaluates, establishes, implements and adapts an individualized discharge plan of care according to the psychological, social, functional, and financial impact of illness on patient/family according to standards of practice, payer source and patient's needs.
  • Appropriately serves as the liaison to the payer source obtaining authorization and updating as needed. Promotes efficient utilization of healthcare services at the most appropriate level of care as determined by professional standards.
  • Establishes and maintains methods of communications with referring and supporting healthcare individuals and organizations to ensure clear and concise exchange of necessary information in a timely manner. Facilitates patient's movement through the program by providing proactive planning and creating solutions to facilitate seamless care. Advocates for patients' ongoing and discharge needs.
  • Coordinates contact and refers to appropriate agencies, services and resources to set up for patients' postdischarge needs including durable medical equipment and followup care.
  • Documents accurately, completely and timely with legible handwriting. Uses measurable and objective terminology, following standards of practice, insurance guidelines and department/hospital policy and procedures.
  • Participates in practice changes as a result of performance improvement activities and changes in healthcare practices. Supports quality standards and initiatives set by the department.
  • Willingly adjusts and adapts to changes in workload by altering working hours to assist in other areas as approved and observed by supervisor.
  • Practices principles of employee and patient safety when performing daily job activities via observation.
  • Utilizes knowledge of growth and development for assigned patient age population. Evaluates for ageappropriate behavior, motor skills and physiological norms of the patient. Interprets data relative to patient age.
Modifies approach and communication skills appropriate to the patient served, which may include:

Neonate/Infant (birth < 1), Pediatric (1 < 12 yrs), Adolescent (12 < 18 yrs), Adult (18 < 65 yrs) and Geriatric (65 yrs).
