    Your Specific Responsibilities:

    Coordinate all administrative day-to-day operations of the MFC Program including maintaining the MFC administrative records and maintaining MFC data and preparing reports. Prepares the MFC in-home record for the MFC parent, coordinate, and track MFC parent in-service training, provide administrative support and guidance for medical foster families. Provide oversight for MFC parent and home requirements and provide technical assistance to MFC parents on those requirements ensure that the MFC Parent Skills Assessments are completed. Observe the condition of the medical home and provide technical assistance and follow up as needed. Performs home or community visits per schedule in MFC Internal Training Manual. Accompany MFC providers and children, as necessary, to appointments and clinics. Review and complete the written evaluation of each MFC home initially, annual and as needed. Maintain administrative record. Complete required documentation on appropriate electronic record. Complies information and prepares reports as required. Assist and encourage the MFC parent to co-parent and to provide the child's birth or adoptive family child-specific training for the child. Ensure that MFC parents are inviting birth and adoptive families to all the children's medical and therapy appointments if appropriate.

    Conducts medical parent support activities including assisting with Medicaid provider rules and regulations, arranging respite, organizing, and providing medical parent support activities, and assisting with obtaining community resources.

    Prepare and participate in MFC staff meetings to provide MFC parent and client updates, trouble-shoot concerns, and develop resolutions to those concerns, completes necessary documentation. Attend and participate CMAT staff meetings. Act as the MFC Program liaison with other community providers and stakeholders, raise community awareness of MFC services. Serve as a liaison between the MFC provider and the MMA Plan. Attends and participates in other meetings as assigned.

    Recruitment and retention of MFC parent providers. Complete in the initial home visit and interview of prospective MFC parents and other pre-training activities. Participate in all local area Medical Foster Parenting pre-service trainings. Coordinate activities related to the on-boarding and pre-service training of prospective MFC parents. Assist MFC parents with Medicaid enrollment processes as requested.

    Participate in court hearings to provide testimony and input to the court, when necessary. Request court orders as needed. Assist with parental and sibling visitation and supervision for MFC children. Conduct initial and monthly statewide MFC bed searches when local MFC beds are unavailable for referred children, as requested by the CBC as directed. Cross train and assume responsibilities of CMAT SW as required or assigned.

    Completes psychosocial assessments for Fee for Service Medicaid clients.

    Other duties as assigned.

    Takes after hours call for the MFC program as assigned.

    Incumbent will have access to records containing social security numbers in the performance of job duties.

    Preferred Qualifications:

    • 1-3 years of social work experience with children
    • Bachelor of Science Degree
