Homecare Specialist:
BRIDGES operates a variety of programs guided by the Philosophy of Independent Living to provide services and supports for individuals with disabilities to experience autonomy in all areas of living_.
BRIDGES acts as a Fiscal Intermediary in a NY State Agency with Choice model, that allows people with disabilities who are Medicaid and long term care eligible to recruit, interview, hire, train and dismiss their own employee known as a Personal Assistant (PA).
QUALIFICATIONS
- Bilingual a plus
- Familiarity with the overall concept of Medicaid and the home care industry
- Familiarity with the Independent Living Philosophy and CDPA
- Capable of communicating with and in a manner that is culturally sensitive to individuals with disabilities
- Must be able to travel locally and regionally for meetings and worker enrollments when needed
- Must be able to maintain solid documentation on all people served
- Excellent interpersonal, organizational, written and oral communication skills; able to interact with all levels of associates in a respectful, confidential manner
- Effective and collaborative team player
- Maintain strong work ethic and strict adherence to code of ethics, standards of conduct and confidentiality requirements.
DUTIES & RESPONSIBILITIES:
- Manage all new referral paperwork to properly onboard caregiver and veteran.
- Communicate with VDS Case manager on any updates or changes in s[pending plan.
- Create and distribute wage notices to Caregivers in the Veteran Directed Services group
- Support homecare team ensuring they stay within their approved budget.
- Assist veterans with hiring care attendants.
- Assist care givers when they have issues with timekeeping, etc.
- Conduct monthly monitoring calls to program participants when needed.
- Prepare and maintain data in compliance with policies and procedures.
- Review veteran's budgets monthly to ensure they are within their budget. Consult with VDC manager when an overage is identified.
- Review team's monthly monitoring reports to ensure they are complete.
- Manage and maintain monitoring schedules to ensure reports are completed by established deadlines.
- Manage and upload documents to shared platforms
- Familiarity with the overall concept of Medicaid and the home care industry
- Maintain agency compliance as presented in the employee handbook.
- Monitor daily entries into the electronic visit verification system
- Maintain strict confidentiality and protect Agency operations by keeping information confidential and secured
- Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies
- Monthly calls to PA's ( Personal assistants under the CDPA program)
- Working knowledge of Microsoft Office Software, including Excel, Word and Power Point.
$41, $43,000.00 per year
Schedule:
- 8 hour shift
- Day Shift (preferred)
- West Nyack, NY required)
