- Completes Level 1 (SCQ-Social Communication Questionnaire) screenings with parent/guardian of clients referred for autism evaluation.
- Coordinates the transition of care between inpatient facilities or referral from external providers.
- Communicates with Client Financial Services to secure insurance approval and authorization prior to services.
- Conducts intake with families, when needed, obtaining parent/guardian consent for service(s) and parent/guardian authorization(s) to release/obtain information.
- Enters necessary information in the electronic medical record and updates information as regularly as needed.
- Provides families with resources for alternative funding sources, community resources, and additional education as needed/requested.
- Communicates with scheduler and therapists on the needs of clients and schedule availability for recommended services.
- Requests verification of benefits and pre-authorization for services.
- Enters case notes in the client's medical record: staffings conducted with therapists, school personnel, family correspondence, and any pertinent information regarding the child served.
- Ensures that prescriptions are obtained prior to evaluation.
- Enters EI authorizations into EHR and attaches authorizations to appropriate appointments.
- Uploads and appropriately classifies authorizations and Early Intervention documentation in EHR.
- Follows up with Early Intervention Service Coordinators on IFSP meeting authorizations that have not been obtained and/or are close to expiration.
- Maintains ASD Scheduling calendar and ensures clinic is scheduled out 8 weeks at a time.
- Coordinates at least three to four clinics a month. Day-of-clinic coordination duties include meeting and greeting families, rooming patients, reviewing the schedule for the day, obtaining additional consent, providing updated forms to the diagnostic team, facilitating team meeting discussions, compiling the family reports, and leading family meetings.
- Coordinates ongoing care for Occupational Therapy, Speech Therapy/Feeding Therapy, Physical Therapy, Autism Diagnosis Clinics, and other programs as assigned.
- Provides Autism New Diagnosis Orientation Training.
- Provides public awareness activities including but not limited to Resource Fairs, educational presentations/training, radio, TV, and interviews.
- Serves as an Easterseals liaison for several community partnerships
- Assists families in identifying and obtaining community resources as necessary.
- Provides parent support and education as requested, including handouts, resources, webinars, and training.
- Provides backup duties for other Care Coordinators as needed.
- Other duties as assigned.
- Bachelor's degree from an accredited university with a degree in Psychology, Social Work, Child Development, or a related field preferred.
- Basic knowledge of early intervention and rehabilitation services.
- A minimum of two years of successful experience working with families.
- Working knowledge of personal computers and software (MS Office Suite).
- Understands and executes the workflow for new evaluations, ongoing treatment, and discharge process.
- Working knowledge of Autism Spectrum Disorders, available intervention strategies, and efficacy of various interventions.
- Working knowledge of community resources and how to access services.
- Proven experience in advanced written documentation and correspondence.
- Excellent use of the English language, especially the rules of syntax, punctuation, and grammar.
- Must have visual ability sufficient to read and complete written correspondence, reports, etc., and computer screen information.
- Frequent use of hands to grasp, handle, or feel.
- Regular standing, walking, sitting, and reaching.
- Ability to lift and/or carry up to twenty-five (25) pounds.
- Ability to drive to all Easterseals centers and service sites within a 100-mile radius, as needed.
- Routinely works in office environments with little exposure to excessive noise, dust, and temperature.
- Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Position Type: Full-time
Position Objective: The Care Coordinator / Autism Clinic Coordinator is charged with leading the coordination of care for the multidisciplinary team and individuals with special needs who are birth-21 years old and their families, including those who will be assessed for autism through our multidisciplinary Autism Diagnostic Clinic. The Coordinator works closely with Scheduling, Client Financial Services, Medical Records, and clinical therapists to ensure appropriate and timely admission, ongoing services, and discharge processes.
