Introduction to the Company
Home & Health Care Management is the oldest home health care agency in Northern California.

Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties.

We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

We provide in-depth orientation, plus one-on-one training in specific skills. We provide supervision for you to obtain a Case Manager Certificate.

You are a Registered Nurse with the State of California, and take pride in adhering to the highest standards in care for patients in their home.


Service Region/ Location
We serve in Butte County and the surrounding areas.


Schedule
We are hiring for both full-time and part-time roles.


Travel
Home health care requires you to drive to patient's homes.

You will need to be in the field approximately 95% of your time, and in the office for the other 5% of your time.


Duties and Responsibilities
To perform this job you must be able to perform the following duties successfully:

  • Develop the patient's individual plan of home care for the safe and effective delivery of home health services. This will include start of care, resumption of care and recertification documentation
  • Provide skilled nursing care in the home of patients
  • Administer medications and treatments as prescribed by physicians and other healthcare providers.
  • Monitor patients' vital signs and respond to changes in their condition.
  • Work closely with an interdisciplinary team such as physicians, nurse practitioners, physical and occupational therapists, and social workers, to coordinate patient care and to achieve optimal patient outcomes
  • Update and educate patients and their families about health conditions, medications, and selfcare techniques specific to their home environment
  • Document all patient interactions and interventions in the electronic medical record in compliance with local, state, and federal laws
  • Provide emotional support to patients and their families while respecting their cultural and religious beliefs
  • Participate in ongoing education and training to maintain knowledge of best practices in home health nursing care.
  • Participate in agency performance improvement activities and continuously stays informed about changes in the field of nursing and home health care
  • Take part in rotating weekend schedule
  • Other duties as assigned

Essential Requirements
Current Registered Nurse in California with a state license

At least one years' experience in home health

Strong clinical assessment, organizational, and communication skills

Ability to work independently and as part of a team.

Provides skilled hands-on home health care to patients in the comfort of their own homes

Current Basic Cardiac Life Support (BCLS)

Ability to pass background check

Eligible to work in the United States

Weekend availability on a rotating basis

Valid California Driver's License

Eligible to be insured under our liability policy with a clean DMV report

Your own vehicle and proof of current auto insurance

COVID vaccinated or have an exemption.


Physical Requirements


The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The work requires physical exertion, and you able to regularly operate moving equipment (driving a car, using a bed, gurney, wheelchair) and use medical equipment.


Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity.

Physical requirement for this role specifically include

  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours
  • Able to lift to 50 pounds without assistance
  • Able to drive intermittently

Compensation
Pay range is $48 to $60 per hour. Exact compensation may vary based on skills, experience, and location.


Benefits
You will be reimbursed for the mileage you travel to patient's homes.

You will receive a monthly phone stipend.

Other benefits are dependent on the number of hours you regularly work each week.

For full-time work, you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA) about 60 days of employment.


Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days of employment.

We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
