- Consult with team members and their managers in customizing software as well as integration with other systems and packages
- Train individuals to operate the packages as well as assist them in the efficient use of the current software.
- Encourages enthusiasm and user participation in clinical computing while stressing the importance of data validity and data security.
- Analyze and evaluate processes related to information flow, and liaise between stakeholders and teams about these processes.
- Acquires a comprehensive knowledge of the software involved to determine what are procedural issues versus system/application deficiencies.
- Serves on home care/case management teams, wherever and whenever management determines the skills and talents of the Coordinator would be best utilized.
- Collects reports on the application and/or system deficiencies and makes suggestions for system improvements and enhanced functionality.
- Coordinates efforts to correct deficiencies and errors that occur in the electronic record.
- Performs other duties as assigned
- One year experience with Electronic Health Records (EHR) systems
- High school diploma or equivalent
- Valid California Driver's License and eligible to be insured under our liability policy with a clean DMV report
- Your own vehicle and proof of current auto insurance
- Above-average interpersonal, problem-solving, and written and oral communication skills while under pressure.
- Ability to work with clerical and administrative staff, as well as health professionals, and collaborate effectively.
- Knowledge of Carevoyant and Matrix software
- Sits, stands, bends, lifts, walks, and moves intermittently during working hours

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.
Overview of the Role
The Clinical Application Coordinator serves as a specialist/integrator in the implementation and ongoing support of multi-service clinical software applications used in the home health care and community-based case management setting.
The Coordinator helps define processes used. They are the subject matter expert, and on-site expert for trouble shooting software and deploying an EHR platform.
The Coordinator speaks with a variety of health care professionals - nurses, therapists, and home health aides – as well as clerical and administrative and billing staff to assist them in applying the EHR system in documenting and processing the care of patients, the billing of health care services and administrative functions to manage the agency.
Responsibilities:
The Coordinator has responsibility for a broad range of activities surrounding the implementation of a comprehensive, standardized integrated health care information system, and associated modules and software systems. Your responsibilities include:

Location:
This is not a remote role and requires you to work in our Chico office.
Schedule:
This is a full-time role, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch.
Travel:
Regular travel is not anticipated for this role. However there may be occasional travel to either the Redding or Sacramento offices, and travel to other locations for training purposes.
Physical Requirements:

Compensation:
Pay range is $25 to $40 per hour. Exact compensation may vary based on skills and experience.
Benefits
You will be reimbursed for the mileage you travel from the office to participant's homes.
You will receive a monthly phone stipend.
On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).
Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.
We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
