    Summary

    Tuberculosis (TB) nurse case manager provides comprehensive case management to TB clients with active or latent disease. Assess, diagnosis, plan, implement and provide health education about tuberculosis and disease transmission, isolation and TB treatment plan.

    Works collaboratively with other members of the multidisciplinary team to assure contact investigation and Directly Observed Therapy (DOT), care coordination, and treatment completion for all TB cases.

    Work Schedule:
    This is a fully benefitted, 1.0 fte that is overtime eligible.

    The typical schedule is Monday thru Friday from Additionally, some of the work associated with this position may be performed through teleworking and will report to an office location and meetings as needed.

    Travel to other work locations may be necessary.

    Employees must reside in Washington state and within a reasonable distance to their King County worksite to respond to workplace reporting requirements.

    Job Duties


    • Managing active TB and latent TB cases.
    • Conduct home/hospital/field visits to assess the client's living situation and provide health education about TB. works with Health Outreach Aide and DRIS to assure DOT and contact investigation are completed according to TB program protocol.
    • Provide input and monitor client progress and response to therapy.
    • Monitor client adherence to the TB treatment regimen.
    • Attend case review to present TB cases to discuss issues and to improve quality of care.
    • Obtain and document health history and make nursing diagnosis based on nursing assessment.
    • Documentation requires electronic charting: EPIC, Tracker.
    • Provide expertise regarding public health nursing scope, practice and policy at many levels.

    Experience, Qualifications, Knowledge, Skills


    Requirements:

    • Bachelor's degree in nursing science or a graduate level nursing degree of MSN, DNP or PhD from an accredited school of nursing.
    • Current Washington State license as a Registered Nurse or Multi-State Licensure.
    • Documentation of current BLS-CPR for Health Care Providers. CPR/AED HeartSaver courses do not meet this requirement.
    • Washington state driver's license or ability to travel to sites within the county.
