- Provide community-based trauma-informed support to transition-aged youth
- Offer advocacy, case management, and rehabilitation services
- Assist youth in accessing essential community resources
- Conduct risk assessments, crisis intervention, and safety planning
- Help youth access necessary services and support
- Deliver services in the community
- Bachelor's degree in Psychology, Criminal Justice, Social Work, or related field preferred
- Minimum 2 years of experience working with system-involved youth
- Experience with case management and rehabilitation services
- Ability to work independently and collaboratively within diverse teams and settings
- Familiarity with county resources and proficiency in documentation and billing

The Team-Based program named EMPOWR aims to support transition-aged youth by providing mental health support, housing stability, employment and education assistance, and community connections through community-based therapy and case management.
The Case Manager role involves offering direct support to the program's youth, collaborating with the EMPOWR Team to provide advocacy, rehabilitation services, and referrals.
