Performance Expectations (Quality and Quantity):

Lead the complete implementation, management, planning, and coordination of a thorough Guardianship Representative Program for Phoenix Counseling Center. This involves regular contact with the wards to ensure their needs are met, extensive case monitoring, counseling, and decision-making on personal welfare and finances. Maintain contact with wards at least every 30 days via phone or on-site visits, and have face-to-face meetings with each ward every 90 days. Develop annual care plans with treatment goals and progress updates.

Accept the assigned caseload willingly and build strong relationships with the wards to enhance service delivery quality. Utilize knowledge of administrative regulations, codes, and laws effectively. Display professionalism in handling cases and maintain positive work relationships within the organization.

Monitor the well-being of the wards, participate in treatment team meetings, ensure service implementation meets wards' needs, and provide advocacy. Represent the Executive Director in various settings and adhere to the organization's ethical standards.

Must possess a solid understanding of guardianship laws, behavior management techniques, family dynamics, intervention methods, and community resources. Should exhibit good judgment, initiative, discretion, ability to work under pressure, meet deadlines independently, and maintain accurate documentation.

Complete assigned duties as directed by the supervisor, attend meetings with various stakeholders, and comply with the organization's Employee Ethics Statement.

General Knowledge and Skills (will possess at time of employment or will be expected to learn upon employment): High school diploma or GED and 6 years MH/SA/IDD experience is required. Must have knowledge of laws, regulations, and policies related to guardianship, behavior management, family dynamics, and intervention techniques. Demonstrated ability to work independently, meet deadlines, and maintain accurate records is crucial.

Education/Experience: High school diploma or GED and 6 years MH/SA/IDD experience. .

Physical Requirements: This role involves standing, walking, bending, and lifting with low physical exertion, as well as regular talking and hearing. These physical demands are expected 80% of the time.

Additional Job Requirements:

Effective communication skills are essential for interacting with colleagues, the public, and individuals served. On-call availability 24/7, reliable transportation, driver's license, and adequate automobile insurance are mandatory. Travel is an integral part of the role, and proficiency in operating office equipment is required.
Written communication skills should be clear and understandable. Ability to work in a high-stress, team-driven environment is necessary.
