The Community Support Specialist (CSS):
Provides one-on-one services for adults and/or children with behavioral health disorders; providing training and support that promote recovery. Services are primarily provided in the community
Conducts assessments of persons served, completes intakes and develops individual service plans
Monitors the following community living areas: housing, entitlements, education, finances, social/recreational, family living, mental and physical health, legal assistance
Provides and documents job mandated services to meet current program budgetary goals
Links persons served to needed services and acts as an advocate as needed
Creates, develops, and maintains linkages with other service providers and community resources that may be of assistance to persons served
Manages all case records in accordance with licensing and contractual requirements using an Electronic Health Record
Develops and maintains positive relationships with funding and regulatory agencies
Flexible schedule
Position Requirements:
Bachelor's Degree in psychology, social work, counseling or sociology required
Experience in the provision of direct client services in a social service related field preferred
Must have safe, reliable transportation and ability to transport persons serviced in personal vehicle within program service area
