    Responsibilities

    The Speech Language Pathologist plays a crucial role in delivering speech therapy services to patients receiving home-based care. As a Speech Language Pathologist, you will:

    • Assess patient needs and establish agreed-upon care goals
    • Deliver speech therapy services in alignment with the care plan
    • Communicate patient status to Clinical Supervisor and referring Physician
    • Ensure accurate documentation of patient progress and care plans
    • Engage in relevant meetings, team discussions, and educational sessions as per organizational requirements
    • Follow OASIS assessment protocols and comply with recertification procedures
    • Implement the 4Ms approach (What Matters, Medications, Mentation, Mobility) and offer Age-Friendly Care

    Qualifications

    • Active State Professional License
    • Valid driver's license and insurance
    • One year of experience as a home care Speech Language Pathologist, proficient in assessment and OASIS-C
    • Demonstrated ability to make informed clinical decisions
    • Strong skills in patient assessment and personalized care planning
    • Proficient in clinical procedures
    • Proficiency in Microsoft Office suite
