    Description

    Case Manager Position at PATH

    PATH is looking for dedicated individuals to join their Site Based Housing team at the El Cerrito office as Case Managers. The organization, People Assisting The Homeless (PATH), has been at the forefront of innovative approaches to help individuals experiencing homelessness in California. Through a range of services like homelessness prevention, employment assistance, and supportive housing, PATH aims to make a positive impact on the lives of those in need.

    Job Responsibilities:

    • Conduct intake and needs assessments
    • Develop Individualized Service Plans
    • Provide referrals and support services
    • Evaluate strengths and challenges

    Qualifications:

    • Analytical skills and attention to detail
    • Collaboration and teamwork abilities
    • Strong communication skills
    • Initiative and creativity in problem-solving
    • Critical thinking skills
    • Adherence to ethics and boundaries

    Requirements:

    • Minimum of two years of relevant experience

    Mandatory Qualifications:

    • All education and experience levels welcome

    Driving Requirements:

    • Valid driver's license and proof of insurance
    • Reliable transportation
    • Ability to meet PATH's insurance criteria

    Employment Eligibility:
