Position Title:
Children's Health Home Case Manager


Position Summary:


Children and Adolescent Case management is a care management program that will ensure the coordination of your child's physical and behavioral health care needs.

By enrolling in a health home, children can receive access to critical services that best meet their needs and support their wellbeing.

A Health Home is a care coordination service model whereby all of an individuals and caregivers communicate with one another so that all of the client's needs are addressed in a comprehensive manner.


Field work required: 65-75 % of the time

RESPONSIBILITIES:


  • Document all case activity, including outreach, consent development and assessment, plan development, client progress and transition arrangements.
  • Administer CANS-NY assessment
  • Develop a comprehensive, familydriven youthguided Plan of Health Care.
  • Provide education and insight to child and family about mental illness or chronic medical conditions.
  • Assist families and children with entitlements (SNAP, Public Assistance, etc)
  • Provide Health Home services for children with medium or high acuity needs, including at least two services per month, with at least one of those conducted face to face.
  • Communicate with all providers involved with child on a monthly bases.
  • Participate in mandatory trainings.
  • Participate in supervision and monthly team meetings.
  • Other duties as may be required.

QUALIFICATIONS:


  • BA with 2 years relevant experience, OR MA with 1 year experience
  • Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
  • Experience coordinating and participating in team settings
  • Solid writing and verbal communication
  • Must be computer savvy
  • Ability to work to travel and work and conduct home or community visits.
