Position Title:
Children's Health Home Case Manager
Position Summary:
Children and Adolescent Case management is a care management program that will ensure the coordination of your child's physical and behavioral health care needs.
By enrolling in a health home, children can receive access to critical services that best meet their needs and support their wellbeing.
A Health Home is a care coordination service model whereby all of an individuals and caregivers communicate with one another so that all of the client's needs are addressed in a comprehensive manner.
Field work required: 65-75 % of the time
RESPONSIBILITIES:
- Document all case activity, including outreach, consent development and assessment, plan development, client progress and transition arrangements.
- Administer CANS-NY assessment
- Develop a comprehensive, familydriven youthguided Plan of Health Care.
- Provide education and insight to child and family about mental illness or chronic medical conditions.
- Assist families and children with entitlements (SNAP, Public Assistance, etc)
- Provide Health Home services for children with medium or high acuity needs, including at least two services per month, with at least one of those conducted face to face.
- Communicate with all providers involved with child on a monthly bases.
- Participate in mandatory trainings.
- Participate in supervision and monthly team meetings.
- Other duties as may be required.
QUALIFICATIONS:
- BA with 2 years relevant experience, OR MA with 1 year experience
- Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
- Experience coordinating and participating in team settings
- Solid writing and verbal communication
- Must be computer savvy
- Ability to work to travel and work and conduct home or community visits.

Schedule:
- 7 hour shift

- On call every 6 weeks
