The Job


PRFI's Family Connections Program is seeking a full time Family Case Planner to help strengthen, stabilize and maintain family functioning by assisting caretakers through enhanced parenting, meeting children's needs, prevention of abuse/neglect, coping, communication, and insight skills.


What you will be doing:


  • Conduct initial assessment of family needs pertaining placement risk factors in children, problems, needs, capacity to use preventive services, and the development of a preliminary service plan in close coordination with Supervisor.
  • Responsible for Community Outreach Advocacy.
  • Responsible for the provision of adequate case planning, family counseling, and other casework modalities under close coordination of supervision;
  • Responsible for the planning, coordinator, and delivery of subordinate services to the family through the help of the Family Aide when available.
  • Responsible for monitoring linkages with the Community Services Providers, visits to schools and other providers.
  • Responsible for securing the safety of all children served.
  • Facilitate client's use of the psychiatric and psychological services, and the followup of the clinician's recommendations.
  • Responsible for at least two (2) face to face contacts with the family.
  • Responsible for monitoring and close supervision of case management of all preventive services.
  • Timely completion of all FASPS, including weekly submission of progress notes.
  • Perform the basic administrative tasks related to caseload, such as statistics, recordings, FASPS, progress notes and contact logs.
  • Participate weekly individual supervision and team meetings.

Who you are:


  • Knowledge of Family Connections Model a plus.
  • Interest in working with people with social service needs.
  • Exceptional communication and organizational skills, including excellent phone demeanor and direct communication skills with program participants, vendors and staff.
  • Ability to succeed in a fastpaced deadlinedriven environment.
  • Maintain confidentiality and have the ability to exercise a high level of judgment/discretion.
  • Ability to create and maintain wellorganized administrative and operational systems.
  • Must be able to work independently and manage multiple tasks in a fastpaced environment.
  • Ability to work collaboratively with a variety of people, as well as an aptitude for teamwork.
  • Ability to use public transportation for travel to serve client needs.

What you will need:


  • BA/BS/BSW Degree (required).
  • Masters (preferred).
  • Knowledge and proficiency of MS Word, Excel, PowerPoint, Connections, PROMIS, and willingness to learn new programs as needed.
  • Must be COVID19 vaccinated and with proof.
  • US work authorization.

Pay:
$49,000.00 per year
