_Job Summary__:

_**Under the supervision of the Rehab Manager and will function within the therapy team, is responsible to assist with scheduling appointments, transporting participants to their appointments in the therapy gym, and provide restorative therapy visits under the direction of a licensed therapists.

Will provide various care services and support the needs of the participant as assigned.


_ PRINCIPAL DUTIES &


RESPONSBILITIES:
_


  • Provide restorative care under the direction of the therapy department in a group or individual setting.
  • Adheres to Care Resources safety requirements and participant care plans regarding use of assistive
devices (e.g., gait belts, lifts, slide boards, walkers, wheelchair, etc.), when performing restorative care

  • Maintains positive communication with other disciplines as appropriate, to facilitate participant care and
scheduling.

  • Assists with inventory and upkeep of Care Resources equipment for participants as requested.
  • Maintain quality documentation of services provided in note and/or equipment log, in a timely manner.
  • Understand participant rights and incorporates them into daily practices.
  • Awareness and always uses universal precautions.
  • Performs other duties as assigned.

_ OTHER DUTIES &


RESPONSIBILITIES:
_


  • Performs a variety of administrative duties such as copying, filing, computer data entry, scheduling as requested.
  • Assist therapy and nursing departments in other areas as requested
  • Attends inservice training, as appropriate, annual mandatory training, CPR, and firstaid training.

_Job Qualifications:_

  • Must have acquired a high school diploma or equivalent.
  • Previous experience with Therapy or Restorative programming is preferred.
  • Previous experience working with a frail or elderly population is preferred.
  • Must be able to multitask, work independently and in a team setting.
  • Must be a self motivator, creative, organized, and should be able to function independently on projects as assigned.
  • Must have current valid Michigan driver's license and insurance.
  • Must maintain current knowledge of and adhere to required regulatory compliance by governing entities, HIPAA and PACE specific regulations.

ABILITIES REQUIRED:
_


  • Must be able to read, write, speak, and understand the English language and follow written and oral instructions.
  • Working knowledge of Universal Precautions.
  • Typing skills with computer background necessary to effectively utilize computer software.
  • Effective verbal and written communication skills and possesses good organizational skills.
  • Willingness to accept additional responsibility.
  • Ability to work effectively and in a collaborative manner with all members of the Interdisciplinary Team.
  • Ability to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds.

_Mental & Physical Demands:
_


  • Pressures related to dealing with the concerns and emotional needs of participants and their families, fulfilling scheduling requirements, etc.
  • Ability to relate to those participants with dementia and/or developmentally disabled.
  • Ability to lift, bend, reach, push, pull and move a minimum of 50lbs.

WORKING CONDITIONS:
_


  • Normal participant care environment with mínimal exposure to communicable diseases when using standard precaution methods.
