Care Manager Assistant Position

Under the guidance of the RN Care Manager, the Care Manager Assistant plays a vital role in providing Care Management services for Hoag Hospital and its affiliates (referred to as the 'Organization').

Implement patient discharge plans
Assist in coordinating clinical utilization review
Make third party payer notifications

The Care Management Assistant collaborates with various stakeholders such as insurance companies, healthcare providers, and transportation agencies. They embody the mission and values of the team and Organization, contributing to continuous quality improvement efforts.

Qualifications

Education, Training, and Experience:

High School diploma or equivalent
Minimum one year of administrative or clerical experience

Skills and Qualifications:

Self-directed individual with strong prioritization and time management skills
Excellent attention to detail and work quality
Effective team player with exceptional communication skills
Proficient in Microsoft Office Suite and healthcare database systems
Basic understanding of medical procedures and terminology

Preferred:

Proficiency in multiple languages

Schedule: Sunday, Monday, Tuesday from 8:00am to 4:30pm
