Working cooperatively as a member of the transplant team, you will be responsible for coordinating all the activities related to pre-and post-transplant patients. Theseactivities include evaluating potential recipients and living donors; educatingpotential recipients, living donors, and post-transplant patients; providingconsultative services and education to nursing services during the transplantrecipients hospitalization; and coordinating follow-up care of the transplantpatient. The St. Davids HealthCare Registered Nurse (SDHRN) will fulfill an importantrole as a central patient care provider responsible for the coordination anddelivery of patient centered nursing care. As a full partner of the healthcareteam, you will collaborate with all other healthcare disciplines to achievesuccessful execution of the patients plan-of-care. The SDHRN will utilize physical assessment skillsand advanced diagnostic technology to aid in the assessment of the patientsnursing care needs. Data collected is analyzed, synthesized and disseminated toother healthcare providers. The SDHRN will continue to monitor and assess thepatients physical condition, emotional and social needs, continually refiningthe plan-of-care to ensure achievement of the desired goals of the patient andfacility outcomes.

Qualifications
EXPERIENCE Minimum of two years clinical nursing experience: Required One year critical care nursing experience; experience with renal transplantation or dialysis: Preferred Spanish Speaking: PreferredEDUCATION Graduate from an accreditednursing program or current enrollment in an Alternative Entry MSN program: Required Bachelor's or higher level degree: Preferred LICENSE / CERTIFICATIONS
