OF WORK
An Aging Care Manager 2 works with older adults from initial contact with individuals, their families, and professionals through ongoing care management responsibilities.
This includes specialized duties to maintain a caseload of consumers and regularly coordinate services with physicians, community agencies, financial institutions, and more.
In this position you will manage multiple tasks, paying close attention to detail, which include completion of paperwork, work assignments, and meeting deadlines.

- Fulltime employment.
- Work hours are 8:00 AM to 4:30 PM, Monday
- Friday, with a 60minute lunch.
- Oncall hours when required.
- Some out of county overnight travel may be required.
Salary:In some cases, the starting salary may be non-negotiable.
REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1;
or: - Successful completion of the County Social Casework Intern program;
or: - Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field;
or: - An equivalent combination of experience and training.
Other Requirements:
- You must meet the PA Residency Requirement.
- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- A conditional offer of employment will require a drug screening_._
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

$36,367.50 per year
