    Description

    The Clinical Care Team will receive referrals from primary care providers and collaborate with the primary care team to:

    • Collaborate with primary care nurse and providers.
    • Conduct in-person or remote social needs screening/assessment with primary care patients referred by nurse or provider.
    • Coordinate social services resources like housing, clothing, food, mental health services, etc.
    • Work with other social workers to identify patient and community resources.
    • Perform case management activities.
    • Assist in discharge planning, follow-up, and education with hospitals.
    • Help in obtaining patient records from hospitals, specialists, and imaging centers.
    • Compile and maintain a specialty referrals resource list.
    • Support regional primary care-based initiatives with a social work component.
