    Description

    By The Bay Health, a non-profit established in 1975, set the standard for hospice care in the U.S. by emphasizing the role of the patient in making crucial medical decisions. Our wide range of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care, and Pediatric Care.

    Our team approach aims to address practical, social, emotional, and spiritual aspects of care, with the primary goal of enhancing the quality of life for our patients, caregivers, and families.

    We are currently looking for a Registered Nurse to work weekdays and weekend night shifts, either full-time or part-time (3-5 days per week) in San Francisco County and San Mateo County, with occasional work in Marin County.

    The ideal candidate will possess excellent assessment skills and a solid understanding of best practices in Hospice care to handle incoming calls and provide field care as required.

    Essential Duties & Responsibilities include (but are not limited to):

    • Provide ongoing care to patients and families as requested, following agency policies and applicable regulations.
    • Under the After-Hours Nurse's guidance, take care of patients and families by assessing, planning, implementing, and evaluating nursing services according to the care plan.
    • Coordinate with attending physicians and other community resources to report changes and ensure optimal care for the patient and family.
    • Maintain regular communication with Team Leaders regarding patient and family status, as well as any unusual or potentially concerning issues.

    Education/Experience:

    Requires one year of experience as a professional nurse in the last three years or a Bachelor's degree in Nursing from an accredited School of Nursing. Experience in acute care, home health, or hospice settings is preferred.

    Certificates, Licenses, Registrations:

    • Current license as a registered professional nurse in the State of California.
    • Public Health Nursing certification highly desirable.
    • HPNA certification preferred.
    • Current California Driver's License and automobile insurance meeting State requirements if driving.

    Pay Range:

    RN - $65.00 hourly
