- Conduct initial and ongoing assessments of patients' health status, including physical, psychosocial, and environmental factors.
- Develop and implement individualized care plans based on assessment findings, physician orders, and patient goals.
- Provide skilled nursing interventions, including wound care, medication management, IV therapy, and patient education.
- Perform comprehensive medication reconciliations and ensure accurate administration of prescribed medications.
- Monitor patients' vital signs, symptoms, and responses to treatment, and communicate any changes or concerns to the healthcare team.
- Educate patients and their caregivers on disease processes, treatment plans, medication management, and self-care techniques.
- Coordinate and collaborate with physicians, therapists, social workers, and other healthcare professionals to optimize patient outcomes and continuity of care.
- Document all patient care activities, assessments, interventions, and outcomes accurately and timely in electronic medical records.
- Ensure compliance with regulatory requirements, agency policies, and standards of nursing practice.
- Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery and outcomes.
- Provide emotional support and counseling to patients and their families, promoting holistic well-being and quality of life.
- Maintain professionalism, confidentiality, and ethical standards in all interactions with patients, families, and colleagues.
- Graduation from an accredited school of nursing with a current license to practice as a Registered Nurse (RN) in the state of Georgia.
- Minimum of one year of clinical nursing experience, preferably in home health, community health, or a related field.
- CPR certification required; additional certifications such as BLS or wound care certification preferred.
- Strong clinical assessment and critical thinking skills, with the ability to make independent nursing judgments.
- Excellent communication and interpersonal skills, with the ability to establish rapport with diverse patient populations.
- Proficiency in using electronic medical records systems and other healthcare technology.
- Valid driver's license and reliable transportation required.
- Ability to work independently, prioritize tasks, and manage time effectively in a dynamic home care environment.
