- Impact lives by conducting Adult/Geriatric Assessments, medical history reviews, diagnosis, health education, physician referrals, case management referrals, follow-up, and accurate documentation
- Collaborate with physicians, case managers, social workers, family members, and other medical personnel
- Communicate with Primary Care Physician (PCP) for patient education and follow-up
- Build and maintain relationships with a network of advanced practice providers
- Services offered in various settings such as Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree required OR equivalent experience and NP licensure completion

- Current RN and NP or PA licensure in practicing state with prescription authority or eligibility to acquire prescriptive authority
- Board certification in a Matrix-approved specialty by NCCPA, AANP, ANCC, or AACCN
- Valid BLS, ACLS, or CPR certification
- Minimum 1 year experience as a Nurse Practitioner/Physician Assistant. Ability to travel within or out of state
- Proficiency in EMR software and strong computer skills
- Excellent verbal and written communication with patients, clients, and colleagues
- Positive, team-oriented, adaptable, and flexible with change
- Valid state driver's license, car, and ability to travel, meet company's auto insurance standards

Responsibilities
About the Role:
During visits lasting up to one hour, Matrix providers assess a member's current health, medical history, medication adherence, social environment, and other risks. This in-depth evaluation offers valuable insights into a member's overall well-being, which are often missed during standard office visits. The Matrix Comprehensive Health Assessment enhances care quality and helps address multiple care gaps in one visit.
Nurse Practitioner/Physician Assistant - Per Diem - Home Risk Assessments
Type: Per Diem - Flexible
Location: Home, SNF, Telehealth, and various community settings in: Wytheville, VA
Hours: Days, Evenings, Nights, Weekends
Pay: $95.00 or $110.00 per completed Assessment based on county, 25 hours or more/month-Flexible Schedule
Benefits: Per Diem providers receive limited Voluntary Benefits
