- Make a meaningful impact in people's lives by conducting Adult/Geriatric Assessments, medical history reviews, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and detailed documentation as per Matrix guidelines and protocols
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and other medical staff as necessary
- Engage with Primary Care Physician (PCP) for patient education and provide follow-up care
- Join a national network of advanced practice providers to establish and nurture relationships
- Deliver services in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree or equivalent experience and successful NP licensure completion
- Master's Degree or equivalent experience and successful PA licensure completion
- Active RN and NP or PA licensure in state of practice with prescription authority or eligibility to acquire prescriptive authority
- Board certification from NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialization
- Hold current BLS, ACLS or CPR certification
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant. Ability to travel within or out of the state
- Proficient in computer usage and familiar with EMR software
- Demonstrate strong communication skills, both verbal and written, with patients, clients, and colleagues
- Display a positive, outgoing demeanor and team-oriented mindset. Adaptable and comfortable with frequent changes
- Capable of travel, holding a valid state driver's license, able to operate a car, and proof of adequate car insurance coverage for state of residence

During a thorough visit that can last up to one hour, Matrix providers assess a member's current health status, medical background, medication adherence, social surroundings, and other risks. This deep insight into a member's overall well-being is often challenging to achieve during standard office visits. The Matrix Comprehensive Health Assessment enhances care quality and helps in closing multiple care gaps in a single visit.

Location: Home, SNF, Telehealth, and other local settings in Radford, VA
Hours: Days, Evening, Nights, weekend, Flexible Schedule
PAY: $95.00 or $110.00 depending on county per completed Assessment, 25 hours or more/month-Flexible Schedule
Benefits: PRN providers receive non-benefit package except for limited Voluntary Benefits
Mileage Reimbursement provided for all miles with 20 or more completed assessments/month, No show payment for 10 or more scheduled assessments/month
