    Join Our Team as a Behavioral Coordinator at Our Skilled Nursing Facilities

    SUMMARY:

    As a Behavioral Coordinator within our organization, you will play a crucial role in managing and facilitating behavioral health services at skilled nursing facilities. Collaborating with facility staff, healthcare professionals, and our team, your focus will be on ensuring high-quality behavioral healthcare delivery to meet residents' diverse needs. Your commitment to patient well-being and expertise in behavioral health will greatly enhance the overall care quality.

    Responsibilities:

    • Participate in clinical training, case consultations, and weekly in-person meetings with the facility clinical team.
    • Conduct in-person rounding with nursing facility staff to observe and document patient behaviors following CMS guidelines.
    • Assist in implementing behavioral intervention strategies to manage challenging behaviors and encourage positive coping mechanisms for residents.
    • Collaborate closely with facility teams to seamlessly integrate behavioral health services into care plans.
    • Maintain accurate documentation per regulatory standards, company policies, and best practices.
    • Consult with healthcare providers for referrals to specialized behavioral health services when needed.
    • Engage in company initiatives to enhance behavioral health service delivery in skilled nursing facilities.
    • Ensure compliance with healthcare laws and standards.
    • Keep up-to-date with training and certifications relevant to the role.
    • Adhere to reporting standards and company compliance requirements.
    • Perform additional tasks as assigned by your supervisor.

    TRAVEL:

    Travel to assigned nursing facilities is required.

    REQUIREMENTS:

    • Ability to perform essential functions with or without accommodations.
    • Adherence to facility policies, infection control protocols, and safety guidelines.
    • Current TB screening and COVID-19 vaccination if required.
    • Reliable transportation to travel to nursing facilities.

    EDUCATION & EXPERIENCE:

    • High School Diploma or Equivalent required.
    • Preferably an associate or bachelor's degree in psychology, social work, or related field.
    • Minimum of 2 years of experience in a behavioral healthcare setting.

    CERTIFICATES AND LICENSES:

    • CPR & First Aide Certification required.
    • Non-violent Crisis Prevention (CPI) Certification preferred.

    SUPERVISORY RESPONSIBILITIES:

    • None.

    WORKING ENVIRONMENT:

    This role operates mainly within nursing facilities with some administrative duties in an office setting. The job requires interaction with various individuals, so strong communication skills are essential. Efficiently managing electronic health records systems and utilizing technology for patient care are key aspects of this position.

    WORKING CONDITIONS:

    • Requires remaining stationary for extended periods.
    • Involves frequent walking within office and nursing facility.
    • Utilization of technology and office machinery.
    • Ability to lift, bend, and interact with residents.
    • Effective communication and auditory perception skills.
    • Adherence to infection control protocols.
