    Responsibilities:

    The Hospice Aide reports directly to the Director of Patient Care Services and works under the direct supervision of the RN Case Manager and/or Clinical Coordinator to:

    · Assist with personal care of patient, i.e. bathing, feeding, bed making, mouth care, vital signs and transferring

    · Document properly in patient's chart.

    · Keep Clinical Coordinator/Nurse Case Manager informed of condition of patient/family.

    · Observe infection control and safety measures

    · Assist with patient/family teaching concerning patient care and care for the caregiver.

    · Bereavement activities as requested and/or able.

    · Clean and monitor supplies.

    · Provide companionship

    · Assist with laundry, shopping, light housekeeping, meal preparation and other homemaker tasks

    · Attend and participate in staff and patient review meetings.

    · Maintain and uphold the Hospice Code of Ethics

    · Work as an effective Infinity Hospice Care Group
